It’s a quetion of proper post harvest care. Farmers should ensure maize and other grains are dried to 13% moisture content to prevent growth of the aflatoxin causing fungus.
Though aflatoxin is prevalent in most soils with a concentration in certain areas.

Theft of farm produce has also contributed alot to the rise of aflatoxin cases.
Farmers are forced to harvest maize that is not ready and store it in their houses (rather than granaries) otherwise it will be stolen. It therefore gets contaminated because it will rarely stay at the recommended moisture content.
